Job Description - ADULT SOCIAL CARE - Experienced Mental Health Social Worker - Camden Council

Duties:

Care coordination of complex cases under CPA

Developing co-produced care plans with service users

Risk assessment and management

liaison with inpatient units

accurate record keeping and report writing.

Experience:

Must have experience of working with adults with psychosis and complex needs.

Experience of Care Act assessments and reviews

Must have a good knowledge of relevant legislation, policy & procedure including Safeguarding, Mental Health Act, Care Act and Mental Capacity Act
