7-8 months contract with a Local Authority

Job Summary:

The role is based in co-located offices with our colleagues from health, either at the Princess Royal University Hospital (PRUH) or the Single Point of Access (SPA) offices currently in Orpington. One Bromley - working together - South East London CCG (selondonccg.nhs.uk)

The post involves sourcing domcare support packages to enable people to go home from hospital where they have health and social care needs.

You will be required to work closely with our Colleagues from the PRUH and SPA to enable discharge from any hospital in and out of borough where the London Borough of Bromley have a duty to meet needs.

Key Duties/Accountabilities

To discharge the Authority's responsibilities under ASCOF and other relevant legislation with respect to Service Users with disabilities, vulnerabilities, and Eligible care needs.

To ensure the council's eligibility criteria, policies and procedure are followed in relation to service users.

To help create packages of support to prevent unnecessary reception of people into care and to assist people to remain in their own home where possible and/or to move to suitable and appropriate residential care.

To match service user needs to potential placements or providers.

Essential Experience Required:

Experience of using computer packages and undertaking administrative tasks is essential.

Experience of Care Standards and relevant legislation and the CQC registration and approval process is essential.

Experience of negotiating agreements or contracts is essential.

Previous experience of a customer focussed environment is essential.

Evidence of continued professional and personal development is essential.

Essential Qualification Required:

Educated to a high level A levels as a minimum is essential.

Additional information to note:

Working hours: 22 hours per week.

The roles requires a current standard CRB/DBS clearance.

This role will involve weekend working and as an example would cover: Week 1: Monday, Tuesday, Saturday and Sunday. Week 2: Tuesday, Wednesday, Thursday.
